   Sports Parental Permission Form 2010-2011

 

All 6th-8th grade students and parents are encouraged to participate in our sports program.

·         Girl’s basketball is the first sport of the year, and starts after the first few weeks of the school year.

·         Girl’s volleyball starts at the completion of girl’s basketball and ends before Christmas.

·         Boy’s basketball and girl’s cheerleading start in the beginning of January.

·         There is a tournament at the end of each season.

·         A Sports Appreciation Night is held at the conclusion of all of the sports to honor student athletes and coaches.

·         Practice times are set by the coaches according to scheduling and gym availability.


In order to participate in the sports program you must:

·         Pay a $25.00 Booster Fee required for each family and collected at registration.

·         Sign and turn into the school office the Sports Parental Permission Form.

·         Have a doctor complete a physical form on the student and turn into the school office. Current physicals must be less than one calendar year old in order for a student to participate in sports.

Lourdes student athletes and their parents are expected to display good sportsmanship at all times.  Disciplinary action for violation of School, League or Coaches rules by players or parents may result in, but are not limited to the following: loss of game and/or practice time, temporary suspension and/or permanent removal from the sports program, and removal from event.

 

Sports and Booster programs rely solely on volunteers.  Parents of Lourdes athletes are required to volunteer their time at Lourdes hosted sporting events.  You will receive a schedule and be assigned a time and duty.  If you are unable to attend, it is your responsibility to find a replacement.
